First Visit
Our routine first visit will consist of our staff obtaining a medical and dental history, a complete periodontal examination, and radiographs when necessary. Then we will thoroughly evaluate your records, possibly consult with your dentist and/ or physician if necessary, and develop a treatment plan. You generally will be given another appointment for a consultation, where we will discuss your present periodontal status, treatment options, prognosis, time of treatment, and estimated fees. Our goal is to explain what treatment is necessary to obtain and maintain optimal dental health. We encourage you to ask questions, and you may feel free to bring your spouse or a friend to help you make any decisions concerning future treatment. We also encourage you to visit our Web site at www.gumdrs.com for information about our practice and procedures.
If you click on the Forms tab, you will find a Patient Information Form. Please complete both sides and bring it to the first appointment with you. Your appointment time is time that has been reserved exclusively for you. If you are unable to keep that appointment, please notify us at least 48 hours in advance so that other scheduling arrangements can be made.
